1. A Fulgent Test Requisition Form
2. A specimen transport bag
3. A set of specimen containers
   For general testing, either:
   • 1 Fulgent saliva collection kit
   • 2 EDTA purple top tubes
   • 1 buccal/cheek swab

   For somatic testing:

   • slide mailers

   For preimplantation genetic testing

   • sample tubes/rack
4. Additional materials specific to the type of kit requested.
